Maria E. Johnson, MD offers a web-based service, which provides evaluations and management of psychiatric conditions in adults. She strongly recommends that you carefully read the following terms and conditions as these terms apply to any interaction you have with Maria E. Johnson, MD and her
personnel, through the website, over the phone, during video conferences, and through any other method of communication. These Terms and Conditions are subject to change at any time, at the discretion of Maria E. Johnson, MD, and without notice to you. The service provided by Maria E. Johnson, MD is designed to be remote diagnostic evaluations and treatment, and it is different from the in-person services typically provided by a physician. Physicians providing services using a remote connection may not have the same type or amount of information that would normally be available during an in-person visit with the patient. There are aspects of the practice of medicine that sometimes require a physician to personally examine and observe the condition of the patient. Dr. Johnson may not be aware of facts or information that would affect her opinion of your diagnosis. Therefore, a diagnosis is limited by the nature and amount of information available to her. By engaging in her services you acknowledge and agree that you understand this limitation and agree to assume the risk of any such limitation.

The lack of an in-person evaluation may affect Dr.Johnson ’s ability to obtain information important to your diagnosis and therefore affect her diagnosis and management of your condition.

The use of this website is governed by a Privacy Policy. A copy of this policy is posted on the website. Transactions through this website are protected by encryption software to protect the integrity of your personal information. Information sent over the internet outside of Dr. Johnson’s control is at risk of being disclosed at any number of points of contact with computers outside of his system. You must safeguard your own confidential information to the best of your ability. Do not send sensitive information over the internet if you are not satisfied with the level of security offered.

You assume the risk of any undesirable disclosure or unauthorized intrusion resulting from any security breech, and the risk of any interruption or failure of data while using services provided by Dr. Johnson

If you choose to provide her with personal information via email or mail, or by filling out a form and submitting it through the website, she will use that information only for the purposes of providing services. She does not give, share, sell, or transfer any personal information to a third party unless required by law, or unless you provide her written authorization to do so In addition, Maria E. Remote services with Maria E. Johnson, MD are primarily conducted via Skype and FaceTime. Please review the Terms and Conditions and Security Policies of these companies prior to agreeing to their services.

Dr.  Johnson does not bill any form of insurance for her services.  Payment is expected via the Luminello website or Paypal.

All payments are made online at the time an appointment is scheduled. All payments are processed through Paypal or Luminello.

Refunds

Missed appointments or cancellations made with less than a 24 hour notice are non-refundable.

Patients are under no obligation to continue treatment should they decide to terminate at any time. However, if you wish to end treatment prematurely, you are strongly encouraged to discuss your thoughts openly.

Should you wish to contact Dr. Johnson between scheduled sessions, you are encouraged to email.  Dr. Johnson prefers to communicate via email or the Luminello portal.  Dr. Johnson will make every attempt to return messages within one business day.  She will not check email from Friday at 430pm until Monday morning at 9am so please do not expect a response from email sent on the weekend until Monday. Should you experience a life-threatening emergency, call 911 for assistance. You should then notify Dr. Johnson. Every effort will be made to return your call promptly.
